Hello,

I am trying to upload my son’s physical so he can sign up for sports, but the system will not let me complete the process. Each time I hit “Submit,” I am redirected to a blank page that says “BOUND,” and none of the information is saved. I have tried re-entering the forms multiple times, but the same thing happens.

Could you please look into this issue or let me know if there is another way to submit the physical? This is time-sensitive so my son can participate in sports.
